According to the National Kidney Foundation, one in three US adults is at risk of developing chronic kidney disease (CKD). Factors contributing to CKD include diabetes, hypertension, smoking, and obesity. The interdisciplinary care team – which can include other physicians, advanced practice providers, nurses, dietitians, pharmacists, and social workers – has emerged as one solution to improving CKD care. Studies suggest that interprofessional care teams may improve patient education and preparedness prior to kidney failure, both of which have been associated with improved health outcomes. On Wednesday, January 19, 2022, from 2-3 p.m. ET, join Victor Yu, PhD, RD, BC-ADM, and Azeem A. Mohammed, MD, for an interprofessional webinar focused on the collaboration efforts of the care team when treating people with kidney disease. In this in-depth look at CKD, Drs. Yu and Mohammed will provide an overview of the pathophysiology of the kidneys; explain nutrient needs and the importance of nutrition assessments; and discuss not only the roles of the care team members but also how collaboration among the team can result in improved patient health care outcomes and understanding of living with and managing CKD. Participants will engage with health care peers in breakout sessions to discuss strategies and tools to help create, implement, and improve interdisciplinary collaboration. Learning Objectives

About Our Presenters Dr. Mohammed has over six years’ experience in nephrology, with special interest in acid-based disorders, acute renal failure, and continuous renal replacement therapies. In addition to being an exemplary teacher, he is well-published and served as an investigator on multiple clinical trials in the field of acute and chronic dialysis. Dr. Mohammed is board certified in both internal medicine and nephrology by the American Board of Internal Medicine. Dr. Mohammed resides in Atlanta, GA, with his wife and two sons. |
